Subject: Depression
Assalam-O-Alaikum! My name is Rizwan Qadir, 22 years old. My problem is this I am suffering from depression from last five years. My treatment is continuing since last Three years. But still I feel I don’t have any prominent improvement. The main problems are these. I have strong desire to suicide because I think there is nothing in life and I can do nothing. I feel extreme hopelessness, helplessness and worthlessness. My mood is very depressed, mostly remains sadness, very anger and irritable. I feel boredom all the time. I have no interest in anything. My motivation level has very decreased. My sleep is disturbed. I feel I have no ability. I have no decision power. My appetite and weight has increased. I think very negatively. I am socially dependent to others in every matter. I think every thing is bad I cannot live life. Everything seems impossible. I am living existless life. It likes I cannot regain my happiness and good time. I feel I have destroyed my life. My study is discontinuing from three years due to stammering and severe depression now I feel I have very poor concentration and weak memory it is creating troubles for me. How can I improve my concentration? Has there any relation between poor concentration and weak memory. I have many problems due to that I have started to think if you will more you will lose your and family time. You are burden on yourself that’s why I am thinking severely about suicide. I have tried many suicide attempts perhaps 13 but unfortunately survived. I don’t look to run life in great manner. Please read my problem and give answer if there is any solution. I am in severe pain I don’t want to destroy my life please help me. I also want to inform more that I remained admitted in services hospital Lahore twice. I was diagnosed with MSHD, Body Dysmorphic Disorder with Delusional Disorder, Depressive Disorder, Social anxiety and Personality Disorder.
I am writing the medicines that I have taken during last five years.

It sounds as though your depression is quite severe, in addition to being very chronic. I would strongly urge you to seek the advice of a consultant psychiatrist, in order to have all your treatment, both medication and psychotherapy, reviewed. A psychiatrist should be able to give advice as to additional treatment you may seek, and may even recommend inpatient (hospital-based) treatment.
With individuals with long-standing depression, there are several steps that a psychiatrist may suggest. These include increasing the dose of an antidepressant that has been partially successful in the past, or adding another medication (called 'augmentation'). Many individuals who are on antidepressants also find that psychotherapy (talking therapy) provides additional benefits.
Given the presence of five years of symptoms and marked hopelessness, I would strongly advise you to seek out the assistance of a consultant psychiatrist as soon as possible.
